The State Board of Elementary and Secondary Education (BESE) have given Our Lady of the Blessed Sacrament an Approval Classification making us a State Certified Nonpublic School.

ADMISSION POLICY

 

In keeping with our “Declaration of Christian Education” taken from the Documents of Vatican II, St. Paul Edition, p. 239, and in reaffirming the non-discriminatory policy held by the Diocese of Shreveport School System, Our Lady of the Blessed Sacrament Academy admits qualified students to its programs.  It does not discriminate on the basis of sex, race, color, national or ethnic origin or disability, if with reasonable accommodation; the applicant can meet the program requirements.  This school does not discriminate on the basis of race, color, national or ethnic origin or disability in administration of its educational policies, admissions policies, athletic programs, or other programs in accordance with Title VI of the Civil Rights Act of 1964, Title IX of the 1972 Educational Amendments and sections 4.03(a) and 4.03(c) Procedure 75-50.

 

All students, faculty members, and staff, without exception, are admitted to all the rights, privileges, and activities generally accorded or made available at the schools which do not discriminate on the basis of race, sex, color, national, and ethnic origin in the administration of its employment policies, admissions policies, scholarship, loan programs, athletic, and other school-administered programs.

 

Admission Information

As mandated by Louisiana State Law students applying for admission into PK-3 to Kindergarten must meet the required age as indicated below:
For Pre-K3, students must be three years old by September 30 of the year of entrance
For Pre-K4, students must be four years old by September 30 of the year of entrance
For K, students must be five years old by September 30 of the year of entrance
For 1st, students must be six years old by September 30 of the year of entrance

 

Parents of prospective new students in all grades will be required to attend an interview with the school administration.

 

Pre-Kindergarten Programs

For qualifying families financial assistance may be available for our Pre-K Programs. Please note the following:

· Parents of three year olds and kindergarteners may apply for Child Care Assistance. If approved, Child Care Assistance would pay a percentage of the tuition for students enrolled in our PK-3 / Kindergarten Program. Parents would be responsible for the balance of the tuition and all other registration fees. Applications can be obtained from the office.

· Parents of four year olds may apply for enrollment into the FREE State-funded PK-4 Program. If approved by the State, children of the qualifying families, would be eligible to enroll in the program and the State would pay the tuition and registration fees.  Applications for this program can be picked up in January 2010. Parents may monitor the school’s website for notification at www.olbsa.org.

 

Applications for Pre-K 3 and Kindergarten can be obtained from the school office, Monday-Friday, between 8:00 and 3:30p. The completed form with the Non-Refundable and Non-Transferable registration fee must be returned to the Office.

 

Records Required

 

In compliance with the Louisiana State requirement, all students must have a copy of the following information kept in their cumulative folder here at school: a) Immunization Records, b) Social Security Card, and c) Birth Certificate.  All students must also have a completed student information card and a current emergency contact form on file. Catholics must furnish a copy of their baptismal certificate before reception of First Communion.

· The State of Louisiana has the following as its minimum requirements:

· 4 DPT (Diphtheria, Tetanus, Pertussis combined)

· 3 Oral Polio (Last dose must be administered on or after the 4th birthday)

· 2 MMR (after the 1st birthday)

· 1 Rubella (German measles)

· 4 Hib2

· 3 Hepatitis B Vaccine

· Students who are 11-14 years old must take the, adult type Tetanus and Diphtheria
