Admissions Policy

 

In keeping with our “Declaration of Christian Education” taken from the Documents of Vatican II, St. Paul Edition, p. 239, and in reaffirming the non-discriminatory policy held by the Diocese of Shreveport School System, Our Lady of the Blessed Sacrament Academy admits qualified students to its programs.  It does not discriminate on the basis of sex, race, color, national or ethnic origin or disability, if with reasonable accommodation; the applicant can meet the program requirements.  This school does not discriminate on the basis of race, color, national or ethnic origin or disability in administration of its educational policies, admissions policies, athletic programs, or other programs in accordance with Title VI of the Civil Rights Act of 1964, Title IX of the 1972 Educational Amendments and sections 4.03(a) and 4.03(c) Procedure 75-50.

Admission Information
As mandated by Louisiana State Law students applying for admission into PK-3 to 1st grade must meet the required age as indicated below:
·         For Pre-K3, students must be three years old by September 30 of the year of entrance
·         For Pre-K4, students must be four years old by September 30 of the year of entrance
·         For K, students must be five years old by September 30 of the year of entrance
·         For 1st Grade, students must be six years old by September 30 of the year of entrance

 

Parents of prospective new students in all grades will be required to attend an interview with the school administration.

Pre-Kindergarten Programs

For qualifying families financial assistance may be available for our Pre-K Programs. Please note the following:

·         Parents of three year olds may apply for Child Care Assistance. If approved, Child Care Assistance would pay a percentage of the tuition for students enrolled in our PK-3 Program. Parents would be responsible for the balance of the tuition and all other registration fees. Applications can be obtained from the office.

·         Parents of four year olds may apply for enrollment into the State-funded PK-4 Program. If approved by the State, children of the qualifying families, would be eligible to enroll in the program and the State would pay the tuition and registration fees.  Applications for this program will be sent to the school by the State sometime in the Spring. Parents may monitor the school’s website for notification at www.olbsa.org.

 

Applications for grades K-5 can be obtained from the school office. The completed form with the first installment of the registration fee must be returned to the Office.

 

 

 

 

Records Required

 

In compliance with the Louisiana State requirement, all students must have a copy of the following information kept in their cumulative folder: a) Immunization Records, b) Social Security Card, and c) Birth Certificate.  All students must also have a completed census form and a current emergency contact form on file. Catholics must furnish a copy of their baptismal certificate before reception of First Communion.

 

 

 

 

 

 
 


Immunizations Required
  • The State of Louisiana has the following as its minimum requirements:
  • 4 DPT (Diphtheria, Tetanus, Pertussis combined)
  • 3 Oral Polio (Last dose must be administered on or after the 4th birthday)
  • 2 MMR (after the 1st birthday)
  • 1 Rubella (German measles)
  • 4 Hib2
  • 3 Hepatitis B Vaccine
  • Student ages 11-14 year , adult type Tetanus and Diphtheria

Registration Fee Per Child
[Non-Refundable and Non-Transferable]

Our Lady of the Blessed Sacrament Academy will begin registering students on January 7, 2008. The registration fee of $300 is due by March 20th.  After March 20th,  the registration fee is $325.

 

Tuition

All tuition payments are made through the SMART Tuition Management Service ONLY. Tuition payments are not made through the school office at any time.  All tuition payments are due on the 10th of each month and payments begin in June. Tuition must be current before children begin school in August. Students who have delinquent tuition will not be allowed to start school until the tuition is brought current.

 

All other monthly tuition payments must be made on time each month. The payments are due on the 10th of each month. Each family has a ten-day grace period before late fees are instituted by SMART.  Students with delinquent tuition after the ten-day grace period will not be allowed to attend school or take exams until the tuition is brought current.

Note: The month of January, 2009 is designated as Registration Month. The only payment due by the end of the month would be all registration fees.

Eligibility for Financial Assistance

Financial assistance covers tuition only. To apply for tuition assistance, families must be active, registered Catholics in good standing with the Catholic Church and must complete a Catholic Parish Verification Form. Forms are available in the school office. Application is based on the financial need of the family and must be accompanied by evidence and a letter stating the need. Academic work must be satisfactory, demonstrating at least a “C” average for grades 1-5.  School attendance and conduct reports must be satisfactory for requesting, maintaining, and re-applying for financial assistance. The deadline for returning the completed form is March 31, 2008.  It is the sole responsibility of the family to ensure that all required information is included on the form before submitting it. Incomplete forms will not be processed.

 

 

Refer A Family

Refer a family to Our Lady of the Blessed Sacrament Academy. When they complete registration and enroll a new student for the 2008-2009 school year, you will earn a $100 credit that you can redeem during Registration Month, January 2009. To redeem your credit, the new family must be in good standing with the school and remain in the school through January, 2010. The new family must include your name on the application as the person who referred them to our school.

 

Our Lady of the Blessed Sacrament Academy administration reserves the right to deny enrollment and/or require the withdrawal of any child due to parental non-compliance of rules, regulations, policies, and procedures, and misrepresentation of child or family information (i.e. age, health information, past academic records, or church affiliation).
