Application and Interview Process

Students applying for Kindergarten must be five (5) years old by September 30 of the year of entrance. Students entering First Grade must be six (6)
years old by September 30 of the
year of entrance.
·
Application for Kindergarten through Grade 5
can be obtained from the school office.
The completed form with the full registration fee must be returned to
the Office.
·
Parents of prospective new students will be
required to attend an interview with the school administration.
·
Preliminary records required for new students
include a complete medical examination
form, an immunization form, social security number, birth certificate,
baptismal certificate (if applicable), a census form, and an emergency contact
card.

Registration
and Fees
·
Students currently enrolled in Our Lady of the Blessed Sacrament Academy must register in February of each year.
·
The full amount of the registration fee must be paid at the time to guarantee a
seat for the new school year. This
includes book fees and any other incidental fees.
·
Any student who owes delinquent tuition will not be allowed to register until
the tuition is brought current.
·
General registration for the entire
school is held in March. At that time, prospective families are
expected to pay the full amount of the registration fee in order to guarantee a
seat for their children. This includes
book fees and any other incidental fees.
Tuition
·
All
tuition payments are made through the SMART
Tuition Management Service ONLY.
Tuition payments are not made through the school office at any time.
·
The first month’s tuition must be paid by August 10th of each school
year. Students who have delinquent
tuition will not be allowed to start school until the tuition for the first
month is brought current.
·
All other monthly tuition payments must be made on time each
month. The payments are due on the 10th
of each month. Each family has a ten-day
grace period before late fees are instituted by SMART.
·
Students with delinquent tuition after the ten-day grace period will not be allowed to attend
school or take exams until the tuition is brought current.
·
The May
Tuition Payment must be paid by the 20th of the month. Students
with delinquent tuition in May, will not be allowed to take exams or
participate in end-of-the-year ceremonies, which include but not limited to, Field Day, Kindergarten Graduation, 5th
Grade Promotional Exercises, Awards’ Day, etc.
All Kindergarten Tuition and fees must be paid two weeks before
Graduation.

p. 239, Our Lady of the Blessed Sacrament Academy admits qualified students
to its programs.

Family
Education Rights and Privacy Act: The
Buckley Amendment
The Family Educational Rights and Privacy Act
(FERPA) (20 U.S.C. § 123g; 34 CFR Part 99) is a Federal law that protects the
privacy of student education records.
The law applies to all schools that receive funds under an applicable
program of the U.S. Department of Education.
Our Lady of the Blessed Sacrament Academy adheres to the Buckley
Amendment regarding access to student records and academic confidentiality.
